Several separate mechanisms can remove a page: a noindex directive in the HTML or in a response header, a canonical tag naming a different URL, a robots.txt rule preventing the fetch, or the page simply erroring.
They fail differently and they are frequently confused. A robots.txt block stops the fetch but can still leave a URL listed with no description; a noindex removes the result but requires the page to be fetchable in order to be read at all.
